Kingsport City Schools Face Mask Mandate Opt-Out Forms: August 2021

On Friday, August 27, 2021, the Kingsport Board of Education established a face mask mandate for students, employees, and visitors while inside KCS facilities.  This face mask mandate is in effect from August 31 - September 28, 2021. The face mask mandate applies to Pre-K through 12th grade students, staff, and visitors inside all KCS facilities from arrival at school through the end of the instructional school day. KCS employees and visitors may elect to opt-out of the face mask mandate; Exceptions could apply in courses such as wellness and band.

Per Tennessee Governor Bill Lee's Executive Order #84, a student's parent or guardian shall have the right to opt out of any order or requirement for a student in kindergarten through twelfth-grade to wear a face covering at school, on a school bus, or at school functions, by affirmatively notifying KCS.

If you are a KCS parent/guardian and wish to opt-out your child from this requirement, please select your child's school from the list below and complete the form.  A separate form must be filled out for EACH STUDENT.

  • Palmer Center - Teacher
  • Elementary Schools - Homeroom teacher
  • Robinson Middle - 2nd Period teacher
  • Sever Middle - 1st Period teacher
  • High School - 1st Period teacher
